Register Your Child at LifePark

We're excited your family is joining us at The Church at LifePark! You can use this form to register your kids for our programming:

  • Noah's Park (Birth to 4 years old)
  • KidsPark (Kindergarten to 5th grade)
  • WonderPark (specially-abled kids)

Start with your information at the top of this form and then add your kids as "household members."

On Sunday, try to arrive 15 minutes before a service, park in one of our "first time guests" parking spots on All American Boulevard, and come to the first-time kids check in desk. 

I give permission to The Church at LifePark to make or use pictures, slides, digital images, or other reproductions of my minor child(ren), or of material owned by my child and to put the finished pictures, slides, or images to use without compensation in productions, publications, craft projects, website, or other printed electronic materials related to the role and function of The Church at LifePark.
